Job Description Honor Community Health is a 501c Federally Qualified Health Center. Our mission is to provide for the health and wellness needs of the underserved of Oakland County through the provision of comprehensive, integrated primary, behavioral health, and dental care. NOTE: All employees are required to receive the COVID-19 Vaccine Position Description: The Nurse Practitioner is responsible for managing health problems and coordinating high quality primary medical care in partnership and collaboration with Honor's integrated practice team, in support of Honor---s mission of delivery comprehensive integrated care to vulnerable populations of Oakland County in accordance with State and Federal rules and regulations and the nursing standards of care. The FNP will work collaboratively to create and maintain a culture of excellence and dedication to providing compassionate and high quality health care to residents and the community. This position reports to the Director, Integrated Services. Our standard hours are Monday- Friday, 8:30am- 5:00pm. Responsibilities: Under a collaborative agreement with an Honor employed physician, provide patient-orientated, evidence-based, comprehensive, integrated primary care for Honor patients of all ages though out their life cycle for Honor patients, in accordance with Honor policy and procedure, and Honor recognized evidence based practice guidelines. Obtain a complete history, perform a physical examination and provide a professional diagnosis and treatment plan as part of evaluation and management of a patient presenting with specific medical problem(s) or concern(s). Evaluate and treat acute and chronic illness, including developing a treatment plan and assisting with coordination of care. Document patient care encounters (i.e. history and physicals, patient and patient-related communication both during and after hours, etc.) in an accurate and timely manner in the patient---s electronic medical records. Make appropriate referrals to both internal and external specialties within the organization and/or to non-organizational specialties, when such referrals are medically necessary. Provide consultation to other providers, Behavioral Health Consultants (BHCs), and Resident Physicians on as needed basis. Work collaboratively with members of the health care team, including but not limited to Podiatrists, Dentists, Nurse Practitioners (NPs), Behavioral Health Consultants (BHCs), Physician Assistants (PAs), Medical Office Assistants (MOAs), Resident Physicians, Medical Students and physicians from other specialties. Prescribe medication(s) in accordance with Michigan statute and professional practice guidelines. Perform minor surgical procedures. Complete all charts and review and sign-off on all incoming reports (e.g. lab, imaging) and follow-up in a timely manner. In partnership with Honor behavioral health and dental professionals and staff, embrace and apply the Honor integrated primary care model. Assist the CMO in establishing medical policies, and evaluating current practices, policies, and procedures. Participate in meetings such as monthly clinician/provider, all-staff, peer review, quality improvement and assurance meetings, grand rounds, family conferences, huddles, etc. Participate on the Honor Quality Committee; assist in development of quality improvement policies and procedures. Recommend changes in all clinical areas, and/or in general areas to improve patient flow, medical records, billing practices, and appointment practices. Responsible for direction and supervision of clinical staff, as assigned by CMO. Participate in quality improvement initiatives and activities as directed by the Chief Medical Officer (CMO). Participate in the implementation of and follow Meaningful Use and Patient Centered Medical Home guidelines. Participate in peer review process, providing expert and unbiased review of peer physicians and/or mid-level providers. Provide after-hours telephone coverage, as assigned by CMO. Adhere to Honor---s established productivity standards. Develop and foster relationships with various community partners including but not limited to Community Mental Health core provider agencies, local health departments, local hospital networks, etc. Performs other duties as assigned. During a public health emergency, the employee may be required to perform duties similar to but not limited to those in the job description. What are we looking for? Graduate of accredited School of Nursing. Must be a graduate of an accredited Nurse Practitioner program. Must have a current Advanced Practice Registered Nurse (APRN) license. Must possess a current and valid license to practice medicine issued by the State of Michigan. Must possess a current BLS and ACLS certifications. Must be board certified at hire and will be expected to maintain appropriate board certification, licensure, and membership in professional organizations. Eligible to obtain and maintain credentials as a Medicaid and Medicare provider. Must have a valid Michigan Driver---s License with a clean driving record. A minimum of 1-2 years of progressive experience as a Family Nurse Practitioner. Experience with Federally Qualified Health Centers is desired. CPT and ICD coding experience. Knowledge of and experience with electronic health records (EHR) systems.
